Payment Mandate Form

This service allows beneficiaries of the Social Insurance program to securely provide their banking details to facilitate the direct deposit of payments such as the Old Age Pension.

Who Should Use This Service?

This service is intended for individuals who receive payments from the Department of Social Insurance, including Old Age Pension beneficiaries. To ensure timely and accurate payment processing, it is essential to complete the Payment Mandate Form and provide all required documentation.

How to Complete the Payment Mandate Form

To complete the Payment Mandate Form:

  1. Download the Form
     Download the Payment Mandate Form
  2. Fill Out Sections 1 & 2 Only
    Section 1: Beneficiary Information - Provide your full name, address, and date of birth. If applicable, include your UK National Insurance number.
    Section 2: Bank Details - Enter your bank’s name, IBAN/Account number, Swift Code/Routing number, and Sort Code. Ensure that all details are accurate to avoid delays in payment.
  3. Submit the Form - Email Submission: Once the form and all supporting documents are completed, please email them to sibenefits@gov.bm.

Important Notes

Payments will not be authorized if the form is incomplete, illegible, or missing the required documentation.

We do not transfer payments into third-party accounts. The bank account provided must be in the beneficiary’s name.

A processing fee of $19.50 may apply.
