- 4 page single sided passport application form
- Must be in black ink
- Sections required – 1, 2, 3, (a & b if applicable) 4, 5, 8, 9, 10
- Section 4 – if the applicant is born after 31 December 1982, and the parents in section 4 are also born after this date, grandparents details are required on a separate sheet
- Section 6 is required if the child is age 12 – 15 years (note – if the child turns 12 years old within 4 weeks of submission, they must sign)
- Section 8 – tell us which countries passport your countersignatory holds and their email address (only for children 15
- Section 9 – to be completed by the parent or the legal guardian for children under the age of 16 years. All legal guardians must produce a court document. (Note – if the applicant turns 16 years old within 4 week of submission, they must sign section 9,
- Section 10 - the countersignatory must confirm parent’s name

- Applicant’s original birth certificate (if applicable)
- Original registration certificate (if applicable)
- Parent(s) original birth certificate
- Parent(s) original marriage certificate (if applicable)
- Parent(s) original divorce absolute/making of conditional final (if applicable)
- Parent(s) original naturalistion/registration certificate (if applicable)
- Grandparent(s) original birth certificate (if applicable)
- Grandparent(s) original marriage certificate (if applicable)
- Grandparent(s) original divorce absolute/making of conditional final (if applicable)
- Grandparent(s) original naturalistion/registration certificate (if applicable)
- If you hold a passport for any other country, you are required to submit a colour copy of your passport(s) as supporting documents
